Madeira Relaunches Home Support Scheme for Hospital Transition Patients

The Regional Government of Madeira has relaunched its Home Support Caregivers project, enlisting 24 new operational assistants.

These assistants will help patients transition safely from hospital to their homes. The new recruits have undergone theoretical training coordinated by SESARAM’s Social Service and will receive practical training in hospital wards with patients nearing discharge.

The service will operate between 08:00 and 16:00, offering direct assistance combined with social worker monitoring. Support will be provided for 30 days post-discharge to allow families time to reorganize. Regional Secretary for Health and Civil Protection, Micaela Freitas, highlighted the project as an innovative response to Madeira's aging population challenges.
